Good Afternoon:
Please find attached information regarding our 2016 Benefits Open Enrollment. It is important that you read carefully, maintaining an awareness of deadlines which must be met. Unlike last year, the only individuals who must respond are:
- Those who wish to change their benefits plan or change family members being covered.
- Those individuals who have currently declined benefits must complete a new form and
decline again this year. This is a requirement under the Affordable Care Act.
Again, you do not need to complete a new form unless either of the above apply. If you are making changes or continuing to decline benefits, you will find an enrollment form attached.
All individuals who do not complete an enrollment form will have a continuation of their benefits that are currently in place.
Unless you have a qualifying event (birth of child, marriage, etc.) this will be your only opportunity to make changes until next year’s open enrollment. If you are completing an enrollment form, it must be returned to the benefits office by
May 31, 2016.
If you have any questions, please contact the benefits office. You will find our phone numbers in the attached letter.
