Good Morning,
Worker’s Comp: If you are injured at work, please immediately notify your building administration and school nurse. It is IMPERATIVE that you go to the Worknet doctors. The only exception is if you are transported via ambulance to the emergency room, it should be Reading Hospital. The reason this is so important is that if you CHOOSE to go to your doctor instead and it is declared a work related injury, you will be responsible for the cost of treatment. I am attaching the release form that we all signed off on upon hire, but also sign off on upon time of injury.WC 90 day form
